We would also like to enhance the information that can be shared with other NHS partners who are involved in your healthcare, to provide a more integrated approach to care. This can be achieved using something called an Advanced Summary Care Record. It will improve the flow of information across the health and care system and increase safety and efficiency for patients in the NHS. The Advanced Summary Care Record contains the following information from your GP records:
- Significant medical history (past and present)
- Reason for medication
- Anticipatory care information or Advance directives (such as information about the management of long term conditions)
- Communication preferences
- End of life care information
No other information will be available to NHS partners apart from those listed above. We would need your consent before the Advanced Summary Care Record can be enabled from your GP records. We would therefore be grateful if you could contact reception to get a form.

Patients can opt out of the scheme at any time by informing their practice and completing a simple form.
If you choose to opt out of the scheme, then you will need to complete a form and bring it along to the surgery.
